Providing community-based care coordination, education and data tracking network for community health workers, namely, providing an on-line network environment featuring technology that enables users to share data in the field of community-based humanitarian care, specifically, data related to community individuals with modifiable risk factors in the nature of medical, social and behavioral factors, relevant charitable community services available for those individuals, tracking the outcomes of pairing the individuals with these services, and contracting with payers that directly tie payment to those outcomes
